Acts Retirement-Life Communities, Inc. is seeking a Home Health Occupational Therapist to deliver patient-centered OT services in residents' homes across our communities in Florida.

The role focuses on promoting functional independence, safety, and quality of life, with duties including evaluation, care planning, and implementing evidence-based interventions. Acts is currently seeking qualified candidates for our Home Health Occupational Therapist (OT) position.

• In-house program within a welcoming, slower-paced community environment that delivers holistic care with a strong focus on prevention
• Specialized on-site training delivered during paid work hours, some sessions eligible for CEUs
• State-of-the-art rehab gyms and equipment

In this role, you will be responsible for providing occupational therapy services to home health residents/clients in their home environment to promote functional independence, safety, and quality of life. Evaluates residents/clients, develops individualized plans of care, implements evidence-based interventions, and collaborates with the interdisciplinary care team to achieve optimal outcomes in compliance with physician orders, state practice acts, and federal and state home health regulations.

Requirements

The ideal candidate will meet the following requirements:

• Bachelor's degree in Occupational Therapy from an accredited program
• Minimum of 1-2 years of experience in home health is preferred
• Valid Occupational Therapist license or eligibility for license in the state(s) where services are rendered

Pay Range

$76,385.00 - $96,267.00 / year. Starting rate will vary based on skills and experience.
