Job summary

Enhabit Home Health & Hospice in Lincoln, Nebraska seeks an experienced Registered Nurse to lead our home health team as a clinical supervisor. You will coordinate patient care and ensure compliance with standards.

The role emphasizes collaboration with physicians, patients, and families to deliver high-quality home-based care, while maintaining efficient scheduling and care coordination. In-office attendance is essential in a supportive, growth-oriented environment.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical license in state or compact license.
• Minimum two years of nursing experience within last 36 months.
• CPR certification required.
• Valid driver’s license and automobile liability insurance.
• Knowledge of federal, state and local laws governing home care operations.
• In-office attendance is essential; remote work not permitted.

Responsibilities

• Coordinate overall home health services per standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care or evaluation.
• Facilitate relationships between physicians, patients, caregivers and staff.
• Schedule patients to field clinicians and communicate with stakeholders to ensure continuity of care.
