Job summary

Luxury Senior Living Communities in Miami seeks a dedicated Home Health Nurse to coordinate patient care at Palace Suites Independent Living Community. You will review plans, communicate with field staff, and address Medicare issues to ensure high-quality service.

The ideal candidate is an RN with home health exposure, detail-oriented, punctual, and able to multitask under pressure. Weekend and holiday availability is preferred to support residents’ needs.

Qualifications

• RN license required.
• Home health experience or knowledge of clinical terminology preferred.

Responsibilities

• Review admission documentation to ensure service quality.
• Coordinate with field staff to follow policies and procedures.
• Review patient care plans and ensure proper implementation and discussion with patients.
• Maintain daily communication with field staff to meet patient expectations.
• Review physicians' orders obtained by field staff.
• Verify delivery of daily services as scheduled and for urgent visits.

To coordinate the care of our patients and assist with any Medicare issues.

Essential Duties & Responsibilities

• Review all admission documentationin order to ensure the quality of the services to be provided.
• Responsible for coordination withall field staff, that they follow agency's policies and procedures.
• Responsible for reviewing thepatient's plan of care and ensuring that it gets properly implemented,including discussing it with the patients as necessary.
• Maintains daily communication withfield staff in order to ensure that all services are provided as perpatient's expectations.
• Responsible for reviewing allphysicians' orders obtained by field staff.
• Verifies the proper delivery ofdaily services as scheduled, including unscheduled emergent visits inassigned community.

Qualifications

• RN License required.
• Home Health experience of knowledgeof Clinical Terminology preferred.
