• Complete OASIS and discharge planning as needed.
• Document care in the EMR and communicate changes to the physician.
• Educate and supervise patient and family about home care needs.
• Collaborate with supervisor to meet productivity and staffing needs.
• Adhere to compliance, ethical standards, and agency policies.
