Job summary

Interim HealthCare is seeking a Home Health Registered Nurse (RN) in the Kinston/Greenville, NC area. You will provide compassionate, in-home care to patients, coordinate with physicians, and supervise LPNs, CNAs, and aides to ensure quality care.

Qualifications

• Graduate of an accredited nursing program and active RN license.
• Minimum of 2 years nursing experience, ideally in home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing specialty with strong communication and compassion.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases after hospital discharge.
• Collaborate with physicians and oversee LPNs, CNAs, and aides delivering patient care.
• Assess patients, document progress and report changes to physicians.
• Educate patients on plan of care, medications, therapy, diet and exercise.
• Coach families on the plan of care, medications and home safety.
• Coordinate discharge and ensure goals are met.
