Job description

Home Health RN

Summary:
Home Health RNs are responsible for delivering clinical services, including nursing care, therapy, and other health services to patients in their homes. They ensure compliance with healthcare regulations and uphold high-quality care standards.

Reports to: Clinical Manager of Home Health

Compensation: $45-55/hour

Setting: Remote (home office, community)

Key Responsibilities:
  1. Patient Care
    • Complete supervisory visits and 485 POC/OASIS assessments.
    • Implement/receive orders from physicians for patients.
    • Ensure appropriate care as per individual care plans.
    • Monitor patient progress and recommend care plan adjustments.
    • Maintain accurate, timely, and compliant clinical documentation.
  2. Compliance and Regulatory Adherence
    • Ensure agency compliance with healthcare regulations, licensing, accreditation, and certification standards (e.g., Medicare, Medicaid).
    • Conduct internal audits and quality checks.
    • Address patient or regulatory concerns promptly.
  3. Care Coordination and Collaboration
    • Collaborate with interdisciplinary team members to develop and review care plans.
    • Coordinate with family and caregivers to ensure understanding of care needs and discharge plans.
  4. Quality Assurance and Improvement
    • Oversee clinical quality improvement initiatives.
    • Analyze outcomes, identify improvement areas, and implement corrective actions.

Qualifications:

  • Registered nurse with Indiana license (no restrictions).
  • At least two years of nursing experience.
  • Strong interpersonal, relationship-building, and customer service skills.
  • Legal compliance.
  • Positive attitude, independence, and ability to meet deadlines.
  • Organizational skills, attention to detail, problem-solving skills.
  • Excellent communication skills.
  • Dependable vehicle and valid driver’s license.

Physical Requirements:

Regularly: speak, hear, sit, manipulate objects, reach, lift up to 5 lbs.
Frequently: stand, walk, stoop, lift up to 10 lbs.
Occasionally: operate vehicle, climb, lift up to 20 lbs.

Work environment includes office, outside weather, and patient residences.
