Job summary

Your Health is seeking a qualified Nurse Practitioner to service patients in the Savannah area. This provider role performs visits in designated service areas and collaborates with a broad care team to ensure comprehensive management.

It is a full-time salary-based position with the opportunity to receive a monthly bonus. Responsibilities include evaluating high-risk patients, coordinating care plans, documenting care activities, and educating patients.

Qualifications

• Nurse Practitioner, PA, or Physician with license in state of employment.
• Experience in patient care, home health, clinic, facility, telehealth, or specialty care is preferred.
• Strong communication and teamwork skills within interdisciplinary teams.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care activities with the interdisciplinary team to ensure continuity of care.
• Communicate with case managers, coordinators, social workers, pharmacists, and other team members.
• Participate in team meetings to discuss patient progress and care coordination.
• Document care activities accurately to support patient plans and regulatory compliance.
• Provide patient education and self-management resources.
• Support patients and families in understanding healthcare options.
