Job summary

Essen Health Care seeks a Remote House Calls Nurse Practitioner to support our in-home medical services program across several states. You will perform telehealth visits, patient assessments, and care coordination with field clinicians to ensure high-quality remote primary care.

The ideal candidate has an NP license, 2+ years of practice, and comfort with telehealth tools and EMR. This fully remote role emphasizes patient-centered care, quality, and collaboration with interdisciplinary teams.

Qualifications

• Active NP license in the required state(s).
• National NP certification (ANCC or AANP).
• Minimum 2+ years of clinical experience as a Nurse Practitioner.
• Experience in house calls, home health, geriatrics, primary care, or internal medicine strongly preferred.
• Prior telehealth or remote care experience preferred.
• Strong clinical judgment and comfort managing complex patients remotely.
• Excellent communication and documentation skills.
• Must be licensed in NY and one of the other states listed or willing to obtain.

Responsibilities

• Conduct telehealth visits for patients enrolled in the House Calls program
• Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patient requires primary care services
• Perform care management needs assessments, follow-ups, and care plan adjustments
• Review patient histories, labs, imaging, and diagnostic results
• Support medication management and reconciliation
• Coordinate care with primary care providers (if patients already have one), specialists, and care management teams
• Document visits and clinical decisions accurately in the EMR
• Ensure compliance with clinical protocols, quality standards, and regulatory requirements
• Participate in interdisciplinary case reviews and quality improvement initiatives
