Job summary

Girling Health Care of NY in Brooklyn is seeking an experienced and reliable Home Care RN Case Manager to oversee the case management of quality home health services for patients in the community.

You will assess patients, develop care plans, coordinate with physicians and the care team, handle documentation, and ensure visits are authorized and compliant with CMS/DOH guidelines. A NY RN license and CHHA experience are required.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Current NYS RN license required.
• CHHA experience required.
• Minimum of 1 year recent med/surg nursing experience.
• Familiarity with EMR and computer applications (Word, Excel) preferred.

Responsibilities

• Assess and treat adult and geriatric patients in the home setting.
• Plan, implement, manage and evaluate home health services to meet patient needs.
• Update care plans with input from patient, caregiver, physician and health team.
• Complete all required documentation per agency policy in a timely manner.
• Coordinate with physicians and care providers to ensure appropriate care and authorization.
• Supervise and collaborate with the Interdisciplinary Team to meet clinical goals.
• Develop discharge plans in conjunction with the field team and patient/caregiver.
