Job summary

Socket.dev is seeking an RN Case Manager in New York to administer skilled nursing care and supervise the nursing team. You will perform admissions, assess patient conditions, and develop care plans aligned with physician orders to ensure high-quality home health services.

The role requires an active NY RN license, at least one year of home care experience, and strong communication and organizational skills to support patients and families in the home setting.

Qualifications

• Current, valid RN license issued by NYS Department of Education required.
• Minimum one year home care experience preferred.
• Knowledge of State and Federal regulations, payer guidelines.
• Must possess the following skills: Excellent communication and critical thinking skills, flexible and organized, ability to serve as a role model.

Responsibilities

• Provide and coordinate quality patient care, maintain a full caseload and productivity goals with accurate, timely documentation.
• Manage each patient’s care to achieve positive outcomes regardless of payer.
• Oversee the care given by Home Health Aides to assigned patients and supervise nursing staff.
• Develop and implement the nursing care plan.
• Conduct admission and manage care of patient to the agency by obtaining medical history, vital signs, ADL assessment, and home environment evaluation.
• Ensure regulatory requirements and infection control standards are followed.
