Job summary

Health Care Administrators Association® (HCAA) seeks an experienced home health RN to deliver comprehensive nursing services in patients' homes, assess needs, and develop care plans. You will work with an interdisciplinary team to ensure quality care.

The role requires supervising LPNs and home health aides, coordinating referrals, and educating patients to promote independence, with accurate documentation and on-call support as needed.

Qualifications

• Registered nurse with strong clinical assessment and care planning abilities.
• Excellent communication, documentation, and care coordination skills.
• Experience supervising and supporting team members in a home care environment.
• Ability to educate patients effectively and promote independence.

Responsibilities

• Assess patients’ health needs and develop individualized nursing care plans.
• Implement care plans, including referrals to appropriate services and resources.
• Teach patients self-care, rehabilitation strategies, and health maintenance.
• Coordinate with the interdisciplinary care team to ensure continuity of care.
• Supervise LPNs and home health aides, providing guidance and oversight.
• Document patient assessments, interventions, and outcomes accurately.
• Communicate patient progress and care needs to the care team.
• Provide on-call nursing support as assigned.
