Job summary

Pivot Home Health is seeking Licensed Practical Nurses or Registered Nurses to perform intermittent home-visit care across Colorado. The role offers flexible caseloads with no minimums or maximums and travel within your preferred radius, with employment options PRN, part-time, or full-time.

Qualifications

• Colorado license as RN or LPN without stipulations.
• Home health experience preferred.
• Current CPR certification.
• Current First Aid certification.

Responsibilities

• Coordinate with nursing partner to ensure complete coverage of care.
• Appropriately evaluates the patient initially and periodically throughout care.
• Administer medications, injections, IV infusions, wound, catheter, and ostomy care following strict protocols.
• Coordinate care plans with physicians and other healthcare professionals and maintain accurate medical documentation in EMR/EHR systems (Epic, Cerner, Athenahealth).
• Provide education on disease management, medication administration, nutrition, and self-care techniques to patients and families.
• Monitor patients’ progress through diagnostic evaluations and physiological assessments to adjust care plans as needed.
• Ensure compliance with HIPAA regulations and infection control standards during all patient interactions.

Job Description

Requirements

• Licensed in Colorado as a Registered Nurse or Licensed Practical Nurse without stipulations.
• Home health experience preferred

• Possession of a valid state driver’s license with reliable transportation and applicable carinsurance, if transporting patients
