Pivot Home Health is seeking a Licensed Practical Nurse or Registered Nurse for intermittent care visits across Colorado. Qualifications

• Licensed in Colorado as an RN or LPN without restrictions.
• Home health experience preferred.
• Current CPR certification.
• Current First Aid certification.
• Possession of a valid state driver’s license with reliable transportation.

Responsibilities

• Coordinate with nursing partner to ensure complete coverage of care.
• Appropriately evaluates the patient, initially and periodically throughout care.
• Administer medications, injections, IV infusions; perform wound, catheter, and ostomy care following protocols.
• Coordinate care plans with physicians and other healthcare professionals with accurate medical documentation in EMR/EHR systems.
• Provide education on disease management, medication administration, nutrition, and self-care to patients and families.
• Monitor patients’ progress through assessments to adjust care plans.
• Ensure compliance with HIPAA and infection control standards.
