Job summary

Optum Home & Community Care is expanding its HouseCalls program. This field-based role conducts in-home health assessments for Medicare Advantage and other members, focusing on education, gap closure, and collaboration with PCPs to improve outcomes.

Compensation includes base pay plus bonus with a comprehensive benefits package; flexible scheduling is offered and no on-call, weekends, or holidays are required. Travel to patient homes is expected.

Qualifications

• Active ANCC or AANP certification for NP
• Active NCCPA certification for PA
• Active, unrestricted state licensure
• Driver's license and reliable transportation
• Ability to navigate varied home environments

Responsibilities

• Conduct in-home assessments including medical history, vitals and exam
• Document diagnoses for care management and treatment planning
• Communicate findings to PCPs to close gaps in care
• Identify urgent situations and intervene appropriately
• Educate members on diseases, medications and compliance
• Address social determinants of health and provide referrals
