WellBe Senior Medical in Findlay, Ohio seeks a Nurse Practitioner for home-based adult medicine. This ongoing staff role offers autonomy to provide in-home primary care and coordinate with an interdisciplinary team.

You will manage chronic conditions, prevent ER visits, and support seniors in their own homes while benefiting from a $10K sign-on bonus, mileage reimbursement, and full benefits.

Qualifications

• Master’s Degree (NP) required with board certification (ANCC or AANP).
• Active NP license in good standing.
• Valid driver’s license and vehicle for local field travel.

Responsabilités

• Deliver field-based primary care visits and physical exams in patients' homes.
• Create longitudinal care plans for high-risk patients.
• Collaborate with an interdisciplinary team to ensure safe transitions of care.
• Prevent ER visits by addressing acute needs in the home.
