Qualifications

Responsibilities

• Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the patient’s home.
• Manage Chronic Disease: Create longitudinal care plans for high-risk patients.
• Collaborate Across the Continuum: Partner with an interdisciplinary care team to ensure safe care transitions.
• Prevent ER Visits: Treat acute needs in the home to keep seniors healthy and out of hospital.
