Under the direction of the Director of Nursing, you will conduct assessments, provide counseling, coordinate resources, and contribute to individualized care plans. Key Responsibilities • Conduct psychosocial assessments and evaluate home environments. • Provide counseling and support to clients and families coping with illness or life changes. • Assist clients in accessing community, financial, and family resources. • Participate in developing and implementing interdisciplinary care plans. • Document visits and clinical notes in the EMR within required timeframes. • Monitor and report changes in clients’ emotional or social status. • Consult with physicians regarding care plan adjustments. • Coordinate weekly visit schedules with the Director of Nursing. • Participate in staff meetings, in-service education, and agency activities. • Provide monthly supervision to the Social Work Assistant. • Support discharge planning and continuity of care. • Maintain professional development and comply with agency policies. Job Conditions • Requires strong communication skills and the ability to perform assessments in varied home environments. • Occasional bending, lifting (up to 30 lbs.), and assisting with client mobility may be required. • Must maintain current TB testing.
