Job summary

Gardens Home Health in Southern California seeks Registered Nurse (RN) and Licensed Vocational Nurse (LVN) for per diem home health visits in the San Diego area. You will plan, organize, and direct home care services, and work with a compassionate team to meet patient needs in homes and communities.

Ideal candidates have CA RN or LVN license, BLS, and at least 1 year of home health or strong acute care experience.

Qualifications

• California RN or LVN license required.
• BLS - American Heart Association
• 1 year home health experience or strong acute care experience
• Spanish or Mandarin speaking preferred

Responsibilities

• Schedules visits ahead of time, not same day.
• Regularly re-evaluates patient nursing needs and completes head-to-toe assessment and medication reconciliation.
• Initiates plan of care and revises it as patient status changes.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing and related needs.
• Provides health care instructions to patient as appropriate per assessment and care plan.
• Communicates preferences or challenges with agency office as issues arise.
• Identifies discharge planning needs as part of care plan development and implements before discharge.

JOB DESCRIPTION SUMMARY

Title/Position: REGISTERED NURSE- PER DIEM / LICENSED VOCATIONAL NURSE- PER DIEM

COVERAGE AREA: San Diego

Near: La Mesa, Jamul, Coronado, Chula Vista, La Jolla

PREVIOUS HOME HEALTH EXPERIENCE REQUIRED

SPANISH SPEAKING PREFERRED

The Home Health Nurse plans, organizes, and directs home care services and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

• Initiates the plan of care and makes necessary revisions (RN) as patient status and needs change.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and care plan
• Communicates preferences or challenges with agency office as the issues arise
• Identifies discharge planning needs as part of the care plan development and implements before discharge of the patient.

Required qualifications:

• Education to meet certification, license or registration requirement.
• California Registered Nurse License/ California Vocational Nursing License
• National Provider BLS - American Heart Association
• 1-year Home Health experience or strong acute care experience

Preferred qualifications:

• IV and Central lines experience (RN)
• Experience with wound care including use of negative pressure devices.
