Job summary

AccentCare, Inc. is seeking a licensed LVN/LPN to provide direct patient care under the supervision of an RN in Hinds County and across MS locations.

You will document progress, administer medications, and coordinate care for pediatric to geriatric patients. Requirements include an accredited LVN/LPN program graduate, active license in the state, at least 1 year of LVN/LPN experience, CPR certification, and a valid driver’s license for travel.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of operation.
• One year of LVN/LPN work experience.
• Provide nursing visits per physician's treatment and maintain medical records.
• Must possess and maintain valid CPR certification.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Performs technical nursing functions and direct patient care under RN supervision.
• Promotes and maintains optimal client care under RN supervision.
• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities to case manager and home health team.
• Administers prescribed medications and assesses medicines for interactions and adverse effects.
• Re-evaluates client’s nursing needs and reports findings to Case Manager.
• Counsels and instructs client and family in nursing needs.
• Prepares clinical and progress notes and documents timely.
• Documents in patients’ charts per nursing care plan.
• Communicates with home care team and coordinates services.
