Job summary

AccentCare in Forest, MS is hiring a Licensed Vocational Nurse/Licensed Practical Nurse to provide direct patient care under RN supervision. You will perform technical nursing functions, monitor patient progress, and document care while traveling to assigned locations.

The ideal candidate has an accredited LVN/LPN program diploma, current licensure in MS, CPR certification, and at least 1 year of LVN/LPN experience.

Qualifications

  • Graduate of an accredited practical or vocational nursing program.
  • Currently Licensed Practical Nurse in the state of operation.
  • One year of LVN/LPN work experience.
  • Must possess and maintain CPR certification while employed in a clinical role.
  • Must be a licensed driver who can travel to all business locations.

Responsibilities

  • Performs technical nursing functions and direct patient care under RN supervision.
  • Promotes and maintains optimal client care under RN supervision.
  • Maintains a safe patient environment.
  • Identifies and communicates significant abnormalities in patient data to the case manager and home health team.
  • Administers prescribed medications and treatments as permitted by law and reviews patients’ medications for interactions.
  • Re-evaluates patient’s nursing needs and reports findings to Case Manager.
  • Counsels and educates patients and families on nursing needs and care plans.
  • Prepares clinical and progress notes and documents in a timely manner per policy.
  • Documents on patient charts to reflect the nursing care provided.

This position is paid on a per-point basis. The compensation reflected on this posting, is an estimate of annual compensation.
Schedule: M-F 8a-4:30p
On Call: LPNs are asked to help occasionally to assist with standard visits if on call primary and backup are too full on Saturdays.

What You Need To Know

Under the supervision of an RN, the Licensed Vocational Nurse (LVN/LPN) provides direct patient care. The LVN/LPN observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse. This position provides direct patient care to pediatric, adolescent, adult and geriatric patients.

  • Graduate of an accredited practical or vocational nursing program
  • Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
  • One year of LVN/LPN work experience
  • Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver who can travel to all business locations
Responsibilities

As a LVN/LPN, you will:

  • Performs technical nursing functions and direct patient care consistent with the established plan of care and under the supervision of an RN.
  • Promotes and maintain optimal client care under the supervision of an RN.
  • Maintains a safe patient environment.
  • Identifies and communicates significant abnormalities in patients data to case manager and other appropriate home health team members.
  • Administers prescribed medications and treatments as permitted by law. Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician.
  • Re-evaluates client’s nursing needs and reports findings to Case Manager.
  • Counsels and instructs the client and family in meeting nursing and related needs.
  • Prepares clinical and progress notes.
  • Submits all documentation in a timely manner in accordance with agency policy.
  • Documentation on patients chart correlates with nursing care plan.
  • Communicates frequently and works closely with other home care team members.
  • Assists in coordinating services provided to client
