Job summary

AccentCare, Inc. is seeking a Licensed Vocational Nurse (LVN/LPN) for direct home health care in McKinney, TX. This full-time position offers schedule Monday–Friday, 8:00 AM–5:00 PM, with monthly on-call duties and pay ranging from $36.00 to $41.00 per hour, based on experience.

Work is on-site with per-visit compensation and required patient care under RN supervision. The LVN/LPN must be licensed in the state of operation, have CPR certification, and possess a driver's license to travel between

Qualifications

Graduate of an accredited practical or vocational nursing program.
Currently licensed as LVN/LPN in the state of operation.
One year LVN/LPN work experience.
Provide nursing visits as prescribed and maintain medical records.
CPR certification must be valid during employment.
Must have a valid driver's license and be able to travel.

Responsibilities

Perform technical nursing functions and direct patient care under RN supervision.
Promote and maintain optimal client care under RN supervision.
Maintain a safe patient environment.
Communicate significant patient data to the case manager and team.
Administer prescribed medications and monitor for adverse reactions.
Re-evaluate nursing needs and report findings to the Case Manager.
Counsel and instruct client and family on nursing needs.
Prepare clinical notes and progress documentation.
Submit documentation timely per policy.
Coordinate services with the home health team.
