AccentCare in McKinney, TX is seeking a dedicated Licensed Vocational Nurse (LVN/LPN) for full-time home health visits across Greenville and Quinlan areas. Under RN supervision, the LVN delivers direct patient care, documents progress, and coordinates with the home health team.

The role requires a valid LVN/LPN license, CPR certification, at least one year of LVN/LPN experience, and a willingness to travel to multiple locations. Competitive pay and comprehensive benefits.

Qualificações

• Graduate of an accredited practical or vocational nursing program.
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of operation.
• One year of LVN/LPN work experience.
• Ability to provide nursing visits according to the treatment prescribed by the physician and maintain medical records.
• Must possess and maintain CPR certification.
• Must be a licensed driver capable of traveling to all locations.

Responsabilidades

• Performs nursing functions and direct patient care under RN supervision.
• Promotes optimal client care under RN supervision.
• Maintains a safe patient environment.
• Communicates significant abnormalities to the case manager and team.
• Administers prescribed medications and monitors for adverse reactions.
• Re-evaluates nursing needs and reports findings to the Case Manager.
• Counsels and instructs clients and families in nursing needs.
• Prepares clinical notes and progress reports.
• Submits documentation per agency policy.
• Ensures chart documentation matches care plan.
• Communicates with home care team and coordinates services.
