Job summary

AccentCare, Inc. in Longview, TX is seeking a Licensed Vocational Nurse (LVN/LPN) for home health patient visits. Under the supervision of an RN, the LVN/LPN provides direct patient care and documents medical progress for pediatric through geriatric patients.

This is a full-time, on-site role with a Mon-Fri schedule and monthly on-call. Requires TX LVN/LPN license, CPR, and at least one year of LVN/LPN experience. Pay ranges from $33.00 to $36.00 per hour, based on experience.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Currently licensed as an LPN/LVN in the state of operation.
• One year of LVN/LPN work experience.
• Ability to provide nursing visits and maintain medical records.
• Valid CPR certification.
• Licensed driver able to travel to all locations.

Responsibilities

• Performs nursing functions and direct patient care under RN supervision.
• Maintains an optimal client care level under RN supervision.
• Maintains a safe patient environment.
• Identifies significant abnormalities and reports to case manager.
• Administers prescribed medications and evaluates drug therapy.
• Re-evaluates nursing needs and reports findings.
• Counsels and instructs client and family in nursing needs.
• Prepares clinical and progress notes.
• Submits documentation timely per agency policy.
• Documents on patient charts align with care plan.
• Communicates with home care team members.
• Assists in coordinating services for the client.
