The role requires a current LVN license in Texas, with 1 year of nursing experience preferred, particularly in home health. You will travel to patient homes in the Longview area and collaborate with a healthcare team while maintaining HIPAA compliance.

Qualifications

• Current and valid LVN license in Texas.
• Minimum of 1 year nursing experience preferred, ideally in home health or related field.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively in a field setting.
• Reliable transportation and a valid driver’s license.
• CPR certification preferred.

Responsibilities

• Provide skilled nursing care to patients in their homes according to physician orders and care plans.
• Monitor patient health status and report any changes or concerns to the supervising nurse or physician.
• Administer medications, injections, and treatments as prescribed.
• Assist patients with activities of daily living (ADLs) as needed.
• Document patient care accurately and timely in compliance with agency policies and regulatory requirements.
• Educate patients and their families on health management, medication administration, and disease prevention.
• Collaborate with healthcare team members to coordinate patient care and ensure continuity.
• Maintain patient confidentiality and adhere to HIPAA regulations.
