Job summary

Health Link Home Health and Hospice Agency is seeking a Full-Time Licensed Vocational Nurse (LVN) to care for patients across the South Bay, CA. You will collaborate with the Director of Nursing and the clinical team to ensure coordinated, high-quality home health care.

We offer a $10,000 sign-on bonus, competitive pay of $93,000–$115,000 per year DOE, PTO, health and 401(k) with matching, plus vision, life, and pet insurance. A CA LVN license and driver’s license are required.

Qualifications

• Minimum 1 year of nursing experience in a medical setting.
• IV certification and experience with wound care.
• Home health experience recommended but not required.
• Active California LVN license.
• Valid California Driver’s License.

Responsibilities

• Implement the plan of care initiated by the Registered Nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments.
• Participate in coordination of home health services, reporting additional needs to the RN or Clinical Supervisor.
• Use equipment and supplies effectively and responsibly.
• Complete online charting in Kinnser using company-provided devices.
• Follow all company policies and procedures.
• Perform other duties as assigned.
