High Plains Senior Care Home Health is seeking a skilled Home Health LVN for a per-visit role in Amarillo, TX. The LVN will provide nursing care to patients in their homes under RN/physician supervision, with a sign-on bonus of $2,500.

The ideal candidate has Texas LVN licensure, at least one year of home health experience, and strong communication abilities. A valid driver's license and reliable transportation are required to travel to patient homes.

Qualifikationen

• Current LVN license in Texas is required.
• Minimum of one year of home health care experience.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong critical thinking and problem-solving skills.
• Valid driver’s license and reliable transportation.
• Ability to pass a background check and drug screening.
• Wound care 2-3x per week.

Aufgaben

• Assess patient health status and develop care plans.
• Administer medications and treatments as prescribed.
• Monitor vital signs and report changes to RN/physician.
• Provide wound care and nursing interventions as needed.
• Document patient care and progress in EMR.
• Communicate with patients, families, and healthcare providers.
• Participate in interdisciplinary team meetings and conferences.
• Adhere to state/federal regulations and agency policies.
