Job summary

Kona Medical Consulting is seeking a Home Health Field LVN to provide skilled nursing care to patients in their homes across Texas. The ideal candidate is dependable, clinically confident, and enjoys working independently while being supported by a strong clinical team.

You will travel to patient homes, perform wound care and medication administration, conduct assessments, and collaborate with physicians and RNs to ensure quality care.

Qualifications

• Current Texas LVN license.
• 1+ year of nursing experience (Home Health preferred but not required).
• Strong assessment, communication, and clinical skills.
• Reliable transportation, valid driver's license, and auto insurance.
• Familiarity with EMR/mobile charting.
• Compassionate, dependable, and patient-focused.

Responsibilities

• Provide skilled nursing care under the supervision of an RN Case Manager.
• Perform wound care, medication administration, vitals, and disease process teaching.
• Assist with care plan implementation and updates.
• Conduct patient assessments and report changes in condition promptly.
• Collaborate with physicians, RNs, and other team members to ensure quality care.
• Maintain timely, accurate documentation in the EMR.
• Travel to patient homes within an assigned service area.
