WellMed is hiring Licensed Vocational Nurses to provide direct patient care in home health settings across Texas. You will implement plans of care, document meticulously, educate patients and families, and coordinate with the clinical team to ensure comprehensive services.

Qualified candidates have LVN licensure in Texas, CPR, and at least 2 years of home health experience. Expect a flexible schedule and on-call rotation, with competitive hourly pay in the range of $20.00–$36.00.

Formación

• Graduate of an accredited practical nurse or vocational nursing program.
• Current unrestricted Texas LVN license; CPR certification.
• High school diploma or equivalent.
• TB screening and vaccination/immune proof required.
• 2+ years of clinical home health or medical/surgical experience.

Responsabilidades

• Provides direct patient care as defined by the state Nurse Practice Act and agency policies.
• Implements plan of care initiated by the Registered Nurse.
• Documents timely and accurately according to the plan of care.
• Educates patient and family regarding diagnosis and plan of care.
• Coordinates home health services with clinical team members and reporters.
• Adheres to infection control and safety guidelines.
• Maintains daily patient caseload and time-of-care documentation.
• Communicates changes with physicians and documents orders as required.
• Participates in on-call duties and flexible scheduling as needed.
