Job summary

Quality Private Duty Care is seeking an LPN Staff Visiting Nurse to provide hands-on patient care in home health settings across Tennessee. You will follow physician orders, maintain accurate documentation, and coordinate with the RN Team Leader.

This role requires active TN licensure, a current driver’s license, and willingness to participate in on-call and occasional overtime. You’ll work closely with families to ensure safe, quality care.

Qualifications

• Licensed Practical Nurse with active TN licensure.
• Willingness to provide direct patient care and on-call coverage.
• Experience in home health visits preferred.
• Computer data entry and basic documentation skills desirable.

Responsibilities

• Accepts patient care reports and assignments from the RN Team Leader and documents promptly.
• Obtains weekly visiting schedule and conducts visits per plan of care.
• Notify RN Team Leader of schedule changes with advance notice.
• Maintains an efficient visit route and delivers direct patient care.
• Communicate with RN Team Leader, physicians, and families to coordinate care.
• Complete visit notes, care plans, and discharge/transfer documentation.
