Job summary

Tennessee Quality Care hires an experienced Private Duty LPN to deliver direct patient care in home settings across Tennessee. You will implement the RN-driven plan of care, document care activities, and educate patients and families on self-care techniques aligned with diagnosis and care plans.

Required are an LPN/LVN license, CPR certification, and 3+ years of nursing experience, preferably in home health or medical-surgical settings. Transportation is required.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three years nursing experience; home health or medical/surgical preferred.
• Currently licensed as an LPN/LVN in the state.

Responsibilities

• Provide direct patient care per state Nurse Practice Act.
• Follow and implement plan of care initiated by the RN.
• Document accurately and timely per care plan.
• Educate patients and families on self-care.
• Coordinate home health services with other disciplines.
