Job summary

Senior Care Solutions Group in Boca Raton, FL is seeking a Per Diem Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients' homes across our service territory. This part-time, PRN position requires weekend availability and involves collaborating with RNs and the interdisciplinary team to implement the plan of care.

You will perform wound care, medication administration, monitor vital signs, document findings in the EMR, and educate patients and caregivers.

Qualifications

• Current Florida LPN license.
• 1 year clinical nursing experience; home health preferred.
• Familiarity with Medicare guidelines and home health documentation.
• EMR proficiency (Kinnser/WellSky helpful).
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide skilled nursing care under RN supervision per plan of care.
• Perform wound care, med administration, catheter care, vital signs.
• Observe and report patient condition changes to RN/physician promptly.
• Educate patients/caregivers on prescribed care and meds.
• Document care in EMR accurately and timely.
• Collaborate with RNs, therapists, aides, and physicians for coordinated care.
• Travel to patient homes within designated territory.
