Franciscan Alliance, Inc. in Michigan City, IN is seeking a Licensed Practical Nurse (LPN) for home health to implement nursing plans under the supervision of a registered nurse.

You will provide direct patient care at home, assess patient needs, and collaborate with the care team to achieve outcomes. Travel up to 75% to patient homes is required; sign-on bonus available and minimum start pay is $24.83 per hour.

Qualifications

• Associate degree or diploma from an accredited LPN program is required.
• 1 year acute care, long-term care, or home health experience required.
• 2 years home health experience preferred.
• Licensed Practical Nurse (LPN) license with state board required.
• CPR certification (American Heart Association) required.
• Driver’s license required.
• Auto insurance required.

Responsibilities

• Implement nursing plan of care under RN supervision.
• Provide home-based patient care per physician orders and nursing plan of care.
• Collaborate with Home Health RN and multidisciplinary team to meet patient goals.
• Conduct psychosocial assessments and document care accordingly.
• Assist in discharge planning and arrange post-discharge resources.
• Document patient care accurately and timely per policies.

Licensed Practical Nurses (LPN) among the Franciscan team are those who choose to focus on home healthcare are a special group of people that can provide direct patient care. • Implement nursing plan of care at the direction of the registered nurse.

• Provide home-based patient care based on physician orders and the established nursing plan of care.

• Collaborate with Home Health RN and multidisciplinary treatment team to individualize each patient’s treatment needs, to achieve defined goals for improved patient outcomes.

• Conduct patient psychosocial assessments, to explore the physical, psychological, and social aspects of the patient and their conditions, obtain history, and elicit an accurate description of psychosocial needs.

• Contribute to developing discharge plans, identifying the post-discharge needs of the patient, and target appropriate resources for follow-up care, in collaboration with the patient, family and care team.

• Document patient care information, and ensure timely and accurate completion of patient care records in accordance with policies and procedures.

QUALIFICATIONS

• Associate’s Degree or Diploma from Accredited Licensed Practical Nurse Program - Required

• 1 Year Acute Care. Long‑Term Care or Home Health Experience - Required

• 2 years Home Health Experience - Preferred

• Licensed Practical Nurse (LPN)- State Licensing Board - Required

• Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required

• Driver’s License - Required

• Auto Insurance - Required
