Job summary

VitalCaring is seeking a Licensed Practical Nurse (LPN/LVN) for Home Health, PRN, field-based. You will provide direct patient care in patients' homes under RN supervision, delivering treatments, education, and compassionate care.

You must hold an active LPN/LVN license in the state, have CPR, and preferably 1 year of home health or related nursing experience. You will collaborate with an interdisciplinary team and maintain accurate documentation in the EMR.

Qualifications

Active LPN/LVN license in the state of practice.
Graduate of an accredited practical/vocational nursing program.
Current CPR certification.
Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
Strong clinical and observational skills.
Clear communication and patient education abilities.
Ability to follow care plans while reporting changes in condition.
EMR proficiency and valid driver's license.

Responsibilities

Provide direct patient care in accordance with the state Nurse Practice Act.
Implement and support the plan of care developed by the RN.
Deliver skilled nursing services, including treatments, medication support, and wound care.
Monitor patient condition and report changes to the RN and care team.
Educate patients and caregivers on self-care, treatments, and disease management.
Maintain infection control and proper use of equipment and supplies.
Document patient care accurately and timely within the EMR.
Communicate effectively with physicians, nurses, and interdisciplinary team members.
Participate in case conferences, team meetings, and ongoing training.
Support coordinated care by maintaining consistent communication across the care team.
