VitalCaring – Licensed Practical/Vocational Nurse (LPN/LVN) in Home Health provides direct patient care in patients' homes under RN supervision, delivering planned treatments and education to support healing and independence.

This field-based role offers a flexible schedule, one-on-one patient interactions, and collaboration with an interdisciplinary team to ensure safe, high-quality care while maintaining infection control and proper documentation in the EMR.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med‑Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans while reporting changes in condition.
• Basic proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care under RN supervision per plan of care.
• Deliver skilled nursing services including treatments, meds, and wound care.
• Monitor patient conditions and report changes to the RN.
• Educate patients and caregivers on self-care and disease management.
• Maintain infection control and proper use of equipment.
• Document care timely in the EMR.
• Communicate with physicians, nurses, and care team.
• Participate in case conferences and ongoing training.
• Support coordinated care with consistent team communication.
