You will verify care plans, collaborate with physicians and RNs, teach self-care, monitor vitals, reconcile medications, and document timely. A GA LPN/LVN license, CPR, and a valid driver's license are required.

Qualifikationen

• 1+ year of LPN/LVN experience.
• Current LPN license for the state you work in.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Aufgaben

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
