Job summary

DPS Home Health Agency Inc. is seeking a caring LPN to provide skilled nursing care in patients' homes in West Palm Beach, FL. The role works under RN and physician directions, administering treatments, monitoring conditions, and educating patients and families.

Qualifications

Active Florida LPN license
Current CPR/BLS certification
Excellent assessment, communication, and organizational skills
Valid Florida driver's license and reliable transportation
Ability to work independently with compassionate care

Responsibilities

Provide skilled nursing care per plan of care by RN and physician
Administer medications and treatments as ordered
Monitor patient condition and report changes to supervising RN
Provide wound care and other skilled nursing interventions
Educate patients and families on disease management, medications, and home care
Maintain accurate and timely clinical documentation
Coordinate care with RN case manager, physicians, and healthcare team
Ensure compliance with Medicare, state, and company policies

Education

Florida LPN license

CPR/BLS certification
