Help at Home is seeking a Home Health LPN to deliver skilled nursing care under RN direction in the West Palm Beach area. You will implement care plans, educate patients and families, and coordinate with the healthcare team to ensure quality outcomes.

The role requires an active Florida LPN license, CPR/BLS, strong assessment and communication skills, and reliable transportation. Start with a competitive $30/hour and a flexible schedule to support patient-centered care.

Qualifications

• Current Florida LPN license required.
• CPR/BLS certification required.
• Strong assessment, communication, and organizational skills.
• Valid Florida driver’s license and reliable transportation.
• Ability to work independently with compassionate care.

Responsabilités

• Provide skilled nursing care per plan of care established by RN/physician.
• Administer medications and treatments as ordered.
• Monitor patient condition and report changes to RN.
• Provide wound care and other skilled nursing interventions.
• Educate patients and families on disease management and home care.
• Maintain accurate, timely clinical documentation.
• Coordinate care with RN case manager, physicians, and team.
• Ensure compliance with Medicare, state, and company policies.
