Qualifications

• Graduate of a practical nursing program with active LPN/LVN license in Florida.

• Valid CPR certification and a valid driver's license.
• Proof of auto insurance and passing drug screening.
• Three years of home health experience preferred.

Responsibilities

• Participates in interdisciplinary patient assessments.
• Collaborates on care plans with patients, caregivers and RN supervisors.
• Delivers physician-ordered nursing services per patient plan.
• Monitors patient condition and reports changes to RN supervisor.
• Counsels and educates patients and families on healthcare needs.
• Documents progress with timely clinical notes.
• Supports RNs and physicians in procedures.
• Executess the patient’s plan of care under supervision.
• Teaches self-care and follows nursing standards with the team.
