Job summary

VitalCaring Group is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to join our home health team. You will provide direct, hands-on care in patients' homes, implement care plans under supervision, and support the broader clinical team.

This field-based role covers Steubenville, OH and Wheeling WV, offering a flexible schedule, meaningful patient relationships, and opportunities to grow with a purpose-driven organization.

Qualifications

• Active LPN/LVN license in the practicing state.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).

Responsibilities

• Provide direct patient care under the supervision of an RN.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments and wound care.
• Monitor patient condition and report changes to the RN and team.
• Educate patients and caregivers on self-care and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely in the EMR.
• Communicate with physicians, nurses, and interdisciplinary team members.
• Participate in case conferences and ongoing training.
• Support coordinated care with consistent team communication.
