VitalCaring Group is seeking a Licensed Practical Nurse (LPN/LVN) for home health in Steubenville, OH and Wheeling WV. You will deliver one-on-one care, implement care plans, and educate patients in their homes while collaborating with an RN and interdisciplinary team.

You will provide direct patient care, monitor conditions, and support recovery with skilled nursing services, wound care, and timely documentation in the EMR. A flexible, field-based schedule awaits.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).

Responsabilités

• Provide direct patient care in the home under RN supervision.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self-care and disease management.
• Maintain infection control and proper equipment use.
• Document care accurately in the EMR and communicate with the team.
• Participate in team meetings and ongoing training.
