Job summary

Amedisys is looking for a licensed practical nurse to deliver compassionate home health care in Anniston, Alabama. This full-time role features a Monday–Friday schedule with visits to patients in their homes, enabling care where people feel most comfortable.

Responsibilities include verifying plans of care with the RN, coordinating with physicians, teaching patients and caregivers, checking vitals, and performing required procedures with proper documentation and safety standards.

Qualifications

• Minimum 1 year of clinical LPN experience.
• LPN license valid for the state you work in.
• CPR certification currently active.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
