Job summary

Pinnacle Home Care in Florida's Deland area is hiring a Licensed Practical Nurse to provide in-home skilled nursing under RN supervision. You will use clinical expertise to support patient care plans, document visits, and coordinate with the care team.

Ideal candidates have an LPN/LVN license, BLS, 1 year of home health experience, strong communication and time management, and a valid driver’s license with reliable vehicle to travel to patient homes.

Qualifications

• Current LPN or LVN license (state-specific).
• Graduate of an accredited nursing program.
• Valid BLS certification.
• At least 1 year nursing experience (home health or medical/surgical preferred).
• Excellent communication, organizational, and time-management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Adequate hearing, eyesight, and dexterity for physical assessments.
• Able to perform walking, standing, stooping, and occasional lifting or turning of patients.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care per patient care plans.
• Utilize clinical expertise to make informed decisions that support patient care.
• Complete visit reports promptly in EMR to support quality care and regulatory compliance.
• Confirm daily visit schedules to ensure seamless coordination with staff.
• Engage in the agency’s QAPI program as required.
• Adhere to the agency’s process for addressing customer complaints.
• Uphold patient rights per service agreements and admission policies.
• Understand policies regarding patient discharge and reasons for discharge.
