Qualifications

• Active LPN license issued in Massachusetts.
• Minimum 1 year of home health or related healthcare experience.
• Current Basic Life Support (BLS) certification.
• Reliable transportation and willingness to travel to patient homes.
• Proficient with electronic health records (EHR).
• Excellent communication and interpersonal skills.
• Strong compassion and dedication to quality patient care.

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patient vital signs and overall health status.
• Assist with daily living activities such as bathing, grooming, and meal prep.
• Educate patients and families on managing conditions and care plans.
