Job summary

PurposeCare is seeking a Home Health Licensed Practical Nurse to join our team and provide skilled nursing visits in patients' homes across the Dayton area. The role supports flexible scheduling, autonomy in clinical decision-making, and personalized care within a home-based care model.

Candidates must have a current state nursing license and at least 1 year of nursing experience (home health and OASIS experience preferred).

Qualifications

• Current license with the state Board of Nursing.
• 1 year of nursing experience is required - home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing.
• Current Driver's License, vehicle insurance, and/or access to dependable transportation.

Responsibilities

• Provide skilled nursing visits in patients' homes.
• Coordinate with care teams to ensure quality home health services.
• Observe, assess, and document patient conditions and progress.
• Assist with rehab and therapeutic care as needed.
