CareSphere is seeking a Licensed Practical Nurse (LPN) to provide direct home health nursing care in patients' residences under RN supervision. The role emphasizes data collection, education, and coordination with the interdisciplinary team.

The part-time schedule includes Mon/Wed/Thu/Fri 5 PM-8 PM and Tue 2 PM-8 PM, with a $800 sign-on bonus and daily pay options. Comprehensive benefits, paid training, and scheduling flexibility are offered.

Responsibilities

• Data Collection & Plan of Care: Performs ongoing patient visits, collects data across physical, psychosocial, emotional, family support, and environment status for home care appropriateness.
• Intervention: Performs skilled nursing procedures as ordered and educates patients/caregivers on conditions and self-care.
• Evaluation of Outcomes of Care: Documents progress toward goals and updates care plans with RN collaboration.
• Resource Utilization & Compliance: Ensures Medicare/Medicaid and agency compliance and manages supplies per payer requirements.
• Leadership & Professionalism: Maintains HIPAA compliance and participates in QAPI programs.
