Job summary

CareSphere is seeking a Licensed Practical Nurse to provide in-home patient care under RN supervision. You will perform data collection, execute the plan of care, and deliver skilled nursing interventions in patients' residences while ensuring HIPAA and regulatory compliance.

You will educate patients and caregivers, coordinate with the interdisciplinary team, and document care promptly to support quality outcomes and timely communication with the RN and physician.

Qualifications

• 1 year of home health nursing experience required.

Responsibilities

• Performs ongoing patient visits and collects data on physical, psychosocial, emotional, family support, and environmental status for home care.
• Follows the written plan of care developed by the RN in collaboration with the physician and interdisciplinary team.
• Identifies and reports patient care needs based on home visits.
• Assists the RN in developing patient goals and interventions to achieve expected outcomes.
• Assists in coordinating care with other members of the healthcare team.
• Complies with home health documentation and communication standards.
