Qualifications

• Current and unrestricted LPN licensure in state of practice.

Responsibilities

• Provides high quality clinical services within scope and plan of care, in coordination with the health care team.
• Documents patient visits in electronic medical records per regulatory and payer requirements.
• Participates in care coordination, reports needs and changes in condition, and communicates with patient, family and clinicians.
• Function in diverse home settings regardless of client demographics.
