Job summary

TLC Companions Home Healthcare is hiring an LPN to join our home care team in New Jersey. The role focuses on delivering personalized patient care in home settings, under the guidance of RNs and physicians, with an emphasis on safety and compassionate support.

Qualifying candidates will have 1 year of LPN home care experience, an active LPN license, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• 1 year experience as an LPN in home care
• Current state LPN license
• Graduate of an accredited practical nursing program
• CPR certification
• Flexible schedule availability
• Excellent interpersonal skills
• Valid driver’s license and reliable transportation
• Willingness to travel locally

Responsibilities

• Participate in planning and coordination of total patient care with the RN and physician’s plan of care
• Report and record patient condition and care, including signs of change
• Prepare clinical/progress notes and summaries
• Assist patient with activities of daily living and teach self-care techniques
• Provide and maintain a safe environment for the patient
• Assist physician and RN in complex nursing situations
• Follow universal precautions in patient care
• Follow emergency procedures for incidents or changes in condition
