Amedisys Home Health is seeking a licensed practical nurse for the Greater Plymouth territory. This full-time day position involves in-home visits, coordination with RNs and physicians, and patient education to support care at home.

Responsibilities include verifying plans of care, monitoring vital signs, patient and caregiver teaching, medication reconciliation, and timely documentation. A valid LPN license and CPR are required, with reliable transportation and a driver’s license.

Formación

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsabilidades

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them if patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit; perform specialized procedures ordered by physician (e.g., catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
