Job summary

Well Care Health is hiring a compassionate Home Health Licensed Practical Nurse to join our Growing team and provide exceptional care in patients' homes in the Franklin/Nash/Wilson corridor.

You will administer medications, wound care, and other treatments, monitor patients, and develop personalized care plans in collaboration with the care team. A NC LPN license, CPR, and driving a must are required.

Qualifications

• Graduate of an accredited or approved school of practical nursing.
• Current license to practice practical nursing in the State of North Carolina.
• CPR certification required.
• Minimum one year practical nursing experience required; home health experience preferred.
• Must be able to communicate in English and operate necessary patient care equipment.
• Ability to transfer and assist with patient movement and care.
• Ability to assess, plan, implement and evaluate patient care.
• Strong interpersonal skills with patients, families, physicians and staff.
• Ability to read and follow agency policies and procedures.
• Willingness to work variable hours including weekends/holidays as needed.
• Must have valid NC driver's license and an operational vehicle.

Responsibilities

• Provide nursing care to patients in their homes, including medications, wound care, and other treatments.
• Monitor patients’ conditions, report changes to the care team, and help develop personalized care plans.
• Educate patients and families on health management, medications, and self-care techniques.
• Collaborate with the healthcare team to ensure high-quality care for each patient.
